چکیده

purpose: to report a case who had undergone lasik in his right eye and prk in the fellow eye for correction of equivalent myopia but developed progressive post-operative keratectasia only in the right eye. patient and findings: a 30-year-old man had undergone lasik in right eye 3 years ago for correction of refractive error of -1.75  -1.50@48° and prk in left eye 2 years ago for correction of refractive error of -1.00  -1.75@100°. preoperative pachymetry was 447 mm in the right eye and 446 mm in left eye. however, post-operative pachymetry decreased to 341 mm in the right and 384 mm in the left eye, but only the right eye revealed progressive corneal ectasia that led to severe visual loss (20/400). conclusion: prk may be a better choice for surgical correction of low amounts of myopia due to lack of flap creation; a factor that may weaken the anterior corneal structure and subsequently lead to progressive keratectasia. however, it is prudent to avoid any refractive surgery in eyes with central corneal thickness less than 500 mm.

Pregnancy may trigger late onset of keratectasia after LASIK.

To the Editor: It is common knowledge that the biomechanical stability of connective tissue changes during pregnancy, leading to reduced stiffness and increased extensibility. These changes are most probably hormone-induced and, therefore, corneal biomechanics also may change during pregnancy. Five patients who underwent LASIK experienced vision deterioration during pregnancy, which we attribut...

Corneal collagen crosslinking in post-LASIK keratectasia.

BACKGROUND/AIMS To evaluate the effect of corneal collagen crosslinking with riboflavin and UV-A as a treatment option for postlaser in situ keratomileusis keratectasia. METHODS Crosslinking was carried out in 22 eyes of 15 patients with iatrogenic keratectasia. Follow-up, according to a standardised protocol (uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), slit-lamp ex...

Transepithelial PTK/PRK over complicated LASIK flaps.

To the Editor: We read with interest the recent contribution by Dr. William Bond (Bond WI. PRK over incomplete LASIK flap [letter]. J Refract Surg 2000;16:483). We are convinced that some variant of the technique he described, which we think is probably more appropriately referred to as transepithelial PTK/PRK, will eventually become the procedure of choice for the treatment of some patients wh...

Progressive post-LASIK keratectasia: biomechanical instability or chronic disease process?

Progressive post-LASIK keratectasia (PPLK) is a progressive deformation of corneal anatomy that occurs rarely but may have severe consequences. Using the scientific literature and new hypotheses, we attempted to determine whether PPLK is a biomechanical result of laser in situ keratomileusis (LASIK), a chronic disease process affecting individuals predisposed to the condition, or a combination ...

A prospective, contralateral comparison of photorefractive keratectomy (PRK) versus thin-flap LASIK: assessment of visual function

PURPOSE To compare differences in visual acuity, contrast sensitivity, complications, and higher-order ocular aberrations (HOAs) in eyes with stable myopia undergoing either photo-refractive keratectomy (PRK) or thin-flap laser in situ keratomileusis (LASIK) (intended flap thickness of 90 μm) using the VISX Star S4 CustomVue excimer laser and the IntraLase FS60 femtosecond laser at 1, 3, and 6 ...

Primary Open Angle Glaucoma and Post-LASIK Keratectasia

The patient presented herein is a 52-year-old woman suffering from primary open angle glaucoma (POAG) in both eyes. She has no history of systemic disorders and is not on any systemic medications. She underwent laser in situ keratomileusis (LASIK) 9 years ago for refractive error of -5.50-3.00×180 in both eyes, and was diagnosed with glaucoma 7 years afterwards. Her ocular examination when I sa...
